South Yorkshire and Bassetlaw Sustainability and Transformation Plan (STP)

The South Yorkshire and Bassetlaw STP brings together all our clinical commissioners, hospitals, mental health, social care, public health and local authority organisations as well as working closely with voluntary sector partners and patient and public representatives, to develop a 5 year plan for the Health and Social Care system in our area.

The Working Together Partnership Vanguard is a key delivery arm of a number of proposed STP projects. In the development of the STP Working Together is also looked at as a good example of what can be achieved when organisations work more closely together to deliver healthcare.

Commissioners Working Together

The eight Clinical Commissioning Groups (CCGs) in South Yorkshire and North Derbyshire, along with NHS England and Mid Yorkshire CCGs have formed a similar partnership called Commissioners Working Together.

Motivated by the same factors (sustainably of local healthcare, meeting the needs of an ageing population, maximising productivity across all services, ensuring that services are fit for the future, meeting increased patient expectations and further improving patient care, whilst operating within a challenging financial environment) they work together on setting strategies, commissioning and contracting looking at the whole population needs across the Working Together footprint.
